RELAX INTO YOUR MIND, BODY & HEART

Please read through these General Terms & Conditions carefully. When you are ready to move forward with Life Coaching or Massage services, please contact me, via the button below to set up a Free Meet & Greet appointment. 
Prices & Payment:
  • All prices are including local VAT (21%).
  • Sessions can be paid by bank transfer or by Pay Pal.

Rescheduling & Cancellation
  • If the client wishes to change the time of an appointment, the client is asked to notify the coach (via e-mail) as soon as possible, at least 24 hours in advance so the appointment can be rescheduled and the coaching space can be given to someone else. 
  • Cancellation of a booked session by the client, is possible up to 24 hours before the booked appointment. 
  • In case of cancellation by the client less than 24 hours before the appointment, or in case of a no-show, the coach reserves the right to charge 50% of the fee for the booked service. 
  • In case the coach needs to cancel the appointment, the coach will inform the client as soon as possible, so that replacement date can be scheduled.

Life Coaching
  • All information between client and coach is entirely confidential as to the extent that the law allows and will never be disclosed without the clients expressed authorization. It is, of course, impossible to protect the confidentiality of information that is transmitted electronically through devices connected to internet that do not utilize security or encryption protection.
  • Life coaching is NOT counseling, psychotherapy, psychoanalysis or a provider of mental health services. The client will seek professional guidance for legal, medical, financial, business, mental health, or other matters with the appropriate providers. All decisions in these areas are exclusively the responsibility of the client.
  • The client accepts full responsibility and accountability for the actions he/she takes or does not take, results achieved and any consequences that follow. 
  • The coach does not make any guarantee or warranty as to results that may be achieved.
  • The client or coach can choose to discontinue the coaching relationship at any time. 
  • The client is expected to evaluate his of her own progress and when the coaching is not working as the client desires, he/she should immediately inform the coach. 
  • Life coaching Relax into your Mind, Body & Heart is a relationship designed to facilitate personal and spiritual development, the creation and development of personal goals, and to develop a plan for achieving those goals. In coaching the client sets the agenda and is the expert in his/her own life to bring about desired change. Success is based upon the client’s willingness to take smart risks and try new approaches. The client understands the coach is not hired to tell him/her what to do, rather partnering in brainstorming ideas and possibilities.
  • The relationship between coach and client is designed to be direct and challenging. The client can be assured the coach will be honest and straightforward, asking powerful questions to bring about possibilities and awareness.

Privacy

YOUR PRIVACY
As your coach or massage therapist, based in the Netherlands and the European Union, I am required by law to ask for and keep certain information and records of my clients. Your dossier contains information about your mental, physical health and the treatments you receive in my practice. The dossier may also contain other information that is necessary for your treatment, which I may get from other health care providers, if you give me explicit permission to get this information. 

I will do my utmost best to guarantee your privacy. This means that:
  • I will keep your medical information safe to the best of my ability,
  • I will make sure that no unauthorised persons have access to your information. 

As your coach / massage therapist I am the only one who has access to your records. 
I am bound my law to keep your records confidential. 

The information in your dossier may also be used for the following purposes: 
  • To inform other health care providers, for example if the coaching/therapy is finished, or when you are referred to another health care provider. I will only share your information with your explicit consent. 
  • To be used by a coach/therapist representing me in my absence,
  • or for anonymised peer-reviews.
  • A small part of the data in your file will be used for my financial administration so that I - or my bookkeeper - can  send out an invoice. 
If I wish to use your information for any other purposes, I will first inform you and ask for your explicit permission.  

The information in the dossier is saved for 15 years, as required by law.
 
PRIVACY AND YOUR HEALTH INSURANCE BILL 
If you live in the Netherlands, you can ask for a refund for (a part of) the costs of our consultation from your Health Insurance Company. As required by the Health Insurance Companies, your bill will contain some personal information: 
  • Your name, address and town of residence
  • Your date of birth
  • The date of your treatment
  • A short description of your treatment (e.g. Shiatsu treatment)
  • The costs of the consult
